Category Name Cardio-Diabetic / Lipid-Lowering Agent / HMG-CoA Reductase Inhibitor (Statin)
Common Indications ATORVACARE 10 is indicated for the management of dyslipidaemia and the prevention of major cardiovascular events in patients at elevated risk. It is commonly prescribed for:
- Primary hypercholesterolaemia — elevated total cholesterol and LDL (bad) cholesterol levels
- Mixed dyslipidaemia with elevated triglycerides and reduced HDL (good) cholesterol
- Prevention of heart attack (myocardial infarction) in high-risk patients
- Prevention of stroke and other major cardiovascular events
- Reduction of cardiovascular risk in patients with type 2 diabetes, hypertension, or other cardiovascular risk factors
- Familial hypercholesterolaemia — inherited high cholesterol requiring statin therapy
- Secondary prevention of cardiovascular events in patients with established coronary artery disease
- Adjunct to dietary modifications in the long-term management of elevated blood lipid levels
Common Dosings One tablet (10 mg) orally, once daily — with or without food — or as directed by your physician. The dose may be titrated upward to 20 mg, 40 mg, or 80 mg once daily based on individual lipid targets, clinical response, and tolerability as determined by the treating physician. Take at the same time each day — preferably in the evening — to align with the body's natural nocturnal peak in cholesterol synthesis. Do not abruptly discontinue this medication without consulting your doctor. Regular lipid profile monitoring is required during therapy.
Common Side Effects The following side effects may occur during the course of treatment. Consult your doctor if any of these persist or worsen:
- Indigestion or heartburn
- Diarrhoea or loose stools
- Nausea or upset stomach
- Joint pain or arthralgia
- Pain in extremities or limbs
- Urinary tract infection
- Abnormal liver function tests — inform your doctor promptly if detected on routine blood tests
- Muscle pain, tenderness, or weakness (myalgia) — inform your doctor immediately
- Rarely, severe muscle breakdown (rhabdomyolysis) — characterised by intense muscle pain, weakness, and dark or reddish-brown urine — seek immediate medical attention
- Headache or dizziness
- Serious allergic reactions such as facial swelling, difficulty breathing, or severe skin reactions — seek immediate medical attention if severe
Common Interactions Inform your doctor about all medications, supplements, and herbal products you are currently taking. Notable interactions include:
- Strong CYP3A4 Inhibitors (Erythromycin, Clarithromycin, Ketoconazole, Protease Inhibitors): May significantly increase plasma concentrations of Atorvastatin, substantially raising the risk of myopathy and potentially life-threatening rhabdomyolysis — dose reduction and close clinical monitoring are strongly recommended during concurrent use
- Fibrates (Gemfibrozil) & Niacin: Concurrent use may further increase the risk of serious muscle toxicity including myopathy and rhabdomyolysis — combination should be used only under strict medical supervision with regular monitoring of muscle enzymes (CK levels)
- Warfarin: Atorvastatin may alter the anticoagulant effect of Warfarin — regular INR monitoring is recommended during initiation, dose adjustment, or discontinuation of Atorvastatin therapy
- Grapefruit Juice: May significantly increase the plasma concentration of Atorvastatin by inhibiting CYP3A4 enzyme activity in the gut wall, enhancing both its therapeutic effects and the risk of side effects — avoid grapefruit and grapefruit juice during therapy
- Cyclosporine & Immunosuppressants: May markedly increase Atorvastatin exposure and the risk of myopathy — concurrent use requires careful dose limitation and close medical supervision
- Digoxin: Atorvastatin may modestly increase Digoxin plasma levels — regular Digoxin monitoring is recommended during concurrent use
- Oral Contraceptives: Atorvastatin may increase the plasma levels of certain oral contraceptive hormones — inform your doctor if you are on hormonal contraception
Special Instructions
| Condition | Guidance |
|---|---|
| Pregnancy | Highly unsafe during pregnancy. May cause serious harm to the unborn baby including birth defects and pregnancy loss. Strictly contraindicated — cholesterol synthesis is essential for foetal development and statin use during pregnancy poses significant teratogenic risk. Discontinue immediately if pregnancy is detected and consult your doctor. |
| Breastfeeding | Unsafe to use during lactation. Atorvastatin may pass into breast milk and may harm the nursing infant. Consult your doctor and consider a safer alternative during breastfeeding. |
| Alcohol | Use with caution. Chronic or excessive alcohol consumption may increase the risk of liver damage and elevate liver enzymes during Atorvastatin therapy — limit or avoid alcohol during treatment for optimal hepatic safety. |
| Driving & Machinery | Does not usually impair alertness or driving ability under normal conditions of use. Safe to drive unless you experience unusual dizziness or discomfort during therapy. |
| Liver Disease | Use with caution in patients with active liver disease or persistently elevated liver enzymes. Regular liver function monitoring is recommended during therapy — consult your physician before use in the setting of known liver disease. |
| Kidney Disease | Generally safe to use in patients with renal impairment at recommended doses. No significant dose adjustment is typically required — however, monitor for signs of muscle toxicity (myopathy) which may be more pronounced in renal impairment. Consult your physician for individualised guidance. |
Mechanism of Action ATORVACARE 10 contains Atorvastatin, a synthetic third-generation HMG-CoA reductase inhibitor (statin) that provides potent and sustained reduction of blood cholesterol and triglyceride levels through a highly targeted molecular mechanism:
Atorvastatin works by competitively and selectively inhibiting HMG-CoA reductase — the rate-limiting enzyme in the mevalonate pathway responsible for the endogenous biosynthesis of cholesterol in the liver. By blocking this enzyme, Atorvastatin reduces the intrahepatic concentration of cholesterol, which in turn upregulates the expression of LDL receptors on the surface of hepatocytes. These receptors increase the uptake and clearance of circulating LDL cholesterol (bad cholesterol) and VLDL particles from the bloodstream, significantly reducing total cholesterol, LDL cholesterol, and triglyceride levels while modestly increasing HDL cholesterol (good cholesterol) levels.
Beyond its lipid-lowering properties, Atorvastatin also exerts pleiotropic cardiovascular protective effects including improvement of endothelial function, reduction of vascular inflammation, stabilisation of atherosclerotic plaques, and inhibition of platelet aggregation — collectively reducing the risk of atherosclerosis progression, coronary artery disease, myocardial infarction, and stroke.
Instructions to Take
- Take this tablet exactly as prescribed by your doctor
- May be taken with or without food — however, take at the same time each day, preferably in the evening, to align with the body's natural nocturnal peak in hepatic cholesterol synthesis for maximum therapeutic effect
- Swallow the tablet whole with a full glass of plain water — do not crush, split, or chew
- Avoid grapefruit and grapefruit juice throughout the course of therapy as it may significantly increase Atorvastatin blood levels and the risk of side effects
- Report any unexplained muscle pain, tenderness, weakness, or dark-coloured urine to your doctor immediately — these may be early signs of serious muscle toxicity (rhabdomyolysis) requiring prompt medical evaluation
- Attend regular follow-up appointments for lipid profile monitoring, liver function tests, and muscle enzyme (CK) level checks as advised by your physician
- Complement therapy with lifestyle modifications including a heart-healthy low-fat diet, regular physical exercise, smoking cessation, and weight management for optimal cardiovascular benefit
- Do not self-medicate, adjust the dose, or discontinue therapy without medical advice — even if cholesterol levels normalise, long-term therapy is typically required for sustained cardiovascular protection
- If a dose is missed, take it as soon as you remember — however, skip the missed dose if it is almost time for the next scheduled dose. Do not double the dose to make up for a missed one
- Limit or avoid alcohol throughout the course of treatment
- Keep out of reach of children
- Store in a cool, dry place away from direct sunlight and moisture
