- Hypertension with concurrent fluid retention requiring both vasodilation and diuretic action
Common Dosings One tablet (Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg) orally, once daily — with or without food — or as directed by your physician. Take at the same time each day to maintain consistent blood pressure control. The dose may be titrated by the treating physician based on individual clinical response, blood pressure targets, and tolerability. Do not abruptly discontinue this medication without consulting your doctor. Ensure adequate hydration during therapy, particularly in hot weather or during physical exertion, to avoid excessive fluid loss from the diuretic component.

Mechanism of Action TELMOSAR-H combines two active ingredients from complementary pharmacological classes, each working through a distinct mechanism to provide superior and sustained blood pressure control:
Telmisartan (40 mg): A long-acting, selective angiotensin II type 1 (AT1) receptor blocker (ARB) that works by competitively and specifically blocking the binding of angiotensin II — a potent vasoconstrictor hormone — to its AT1 receptors on vascular smooth muscle and the adrenal gland. By blocking these receptors, Telmisartan prevents angiotensin II from causing vasoconstriction, sodium retention, and aldosterone release, resulting in vasodilation, reduced peripheral vascular resistance, and a sustained reduction in blood pressure. Unlike ACE inhibitors, Telmisartan does not inhibit bradykinin breakdown, significantly reducing the risk of dry cough as a side effect
Hydrochlorothiazide (12.5 mg): A thiazide diuretic that works by inhibiting sodium and chloride reabsorption in the distal convoluted tubule of the kidney. This promotes the urinary excretion of sodium, chloride, and water, reducing plasma volume and cardiac preload, thereby lowering blood pressure. Instructions to Take
- Take this tablet exactly as prescribed by your doctor
- May be taken with or without food — however, take it at the same time each day to maintain consistent blood pressure control
- Swallow the tablet whole with a full glass of water — do not crush, split, or chew
- Monitor your blood pressure regularly as advised by your physician and report any unusual fluctuations
- Ensure adequate fluid intake throughout the day to avoid dehydration from the diuretic component — particularly in hot weather or during physical exertion
- Due to the diuretic effect, take this tablet preferably in the morning to avoid disruption of sleep from increased urinary frequency at night
- Do not abruptly stop this medication without consulting your doctor — sudden discontinuation may cause rebound hypertension
- Rise slowly from a sitting or lying position to minimise the risk of dizziness or sudden drop in blood pressure
- Diabetic patients should monitor blood glucose levels closely as Hydrochlorothiazide may affect glucose tolerance
- Do not self-medicate, adjust the dose, or add other blood pressure or diuretic medications without medical advice
- If a dose is missed, take it as soon as you remember — however, skip the missed dose if it is almost time for the next scheduled dose. Do not double the dose to make up for a missed one
- Limit or avoid alcohol throughout the course of treatment
- Keep out of reach of children
- Store in a cool, dry place away from direct sunlight and moisture
